Cruise Vaccination Requirements Explained


Going on a cruise usually involves visiting foreign ports of call. I am often asked by clients if they need any shots or vaccines to visit different countries.

Whether or not you need vaccinations depends heavily on your specific itinerary, but as a cruise travel agent, I know that requirements can shift based on the ports of call. Generally, most cruises do not have mandatory vaccine requirements for boarding, but individual countries may require them for you to go ashore.

Here are the primary things to look out for:

Mandatory Requirements (Entry/Disembarkation)

The most common mandatory requirement is the Yellow Fever certificate.

• Where: Common in parts of South America (like Brazil and Colombia) and Sub-Saharan Africa.  

• The “Transit” Rule: Some countries in the Caribbean or Asia may require proof of vaccination only if your ship previously docked in a “high-risk” country during the same voyage.  

• Documentation: You must carry the “Yellow Card” (International Certificate of Vaccination or Prophylaxis) and ensure it was administered at least 10 days before arrival.  

Emerging Alerts (Polio)

As of early 2026, the CDC has issued Level 2 travel advisories for over 30 countries due to a resurgence of Polio.  

• Affected Areas: Includes regions in Africa, the Middle East, and even parts of Europe (such as Poland and Germany) where wastewater surveillance has shown presence of the virus.  

• Recommendation: Travelers are encouraged to get a one-time adult booster if they haven’t had one.  

Strongly Recommended (Common Ports)

While not “mandatory” for entry, health organizations recommend these based on local conditions at common cruise stops:

• Hepatitis A & Typhoid: Recommended for many ports in Mexico, Central America, and Southeast Asia where food and water contamination is a higher risk.

• Tetanus & Diphtheria: Ensure your routine boosters are up to date, especially for active shore excursions.

• Malaria: While there is no “vaccine” required for entry, if you are doing river cruises (like the Amazon) or certain tropical excursions, you may want to bring anti-malarial medication.

If you’re planning your cruise, the best way to be certain is to:

Check your Cruise Line’s Portal: Most lines (like Carnival or Royal Caribbean) have a “Travel Documentation” section that lists specific health requirements for your exact sailing.

Visit a Travel Clinic or Doctor’s Office: Do this at least 4–6 weeks before you sail, as some vaccines require multiple doses or take time to become effective.  

NOTE

You will find when you visit your doctor or going to donate blood, you will be asked if you have recently traveled outside of the country. (It can be frustrating to want to help by donating blood and get turned away, but the restrictions aren’t a reflection of your health—they are about the “window period” of certain diseases that the standard blood screening tests might miss. Typically it’s only a short time period before you are allowed to donate.)

When answering, tell your doctor/clinic:

• Exactly which countries (and specific regions/cities) you visited.

• The dates you arrived and departed.

• Any activities involving wildlife, fresh water, or rural areas.
